How does Jagex Catch Real World Traders?

I mean, All you do is trade with someone, and they give you items. How do they monitor that? I think that it is bad how easy it is to Real World Trade. I don't really care people do it, and I don't play a lot, but they say it's so hard to do it undetected... I don't think so.
