Can a student like me get into college?

Hi. I'm currently in high school but at the point where considering college is crucial. I've never been a good student, at least in certain area's like Math and Science and even some social studies courses. I'm really strong in English, Art, Health, History, classes like that. I'm very creative that way but I'm a junior and taking 10th grade math and failing it again. I have an IEP and ADD. But other than Math and Science, I get B's and C's. A's are usually hard for me. I just calculated my GPA. Let's just say my eyes popped when I saw it. Enough said. It was bad. I'm angry b/c I just got a letter in the mail about a college, Pratt Uni, which is my dream school. But it's a private school and though an art and design school it still is hard to get into so they say, and high school grades matter a lot in getting accepted. I've slacked off since...like 3rd grade lol. Not going to lie. Math was such a nuisance and always depressed me and always gave me panic attacks I just got frustrated and gave up on school, to angry to try. But I want security and independence in my future, and I want to be happy and do something I love and am good at like writing or photography. I'm very artistic in multipl areas like photography, writing, art, music ect. I'm no Da Vinci but I'm not bad at all. But I'm embarrassingly bad at math and I don't know why! On my report card my math teacher wrote in the commect box: "poor effort. Appears disinterested/apathetic." APATHETIC! I'm so dead basically. I refuse to be a drop out, and I want to do something with my talents. But how can I get into college with a sucky GPA? They say to basically forget going to college if you don't have A's or B's. Am I just screwed completely or is there hope for me after all? Advice please!
